Job Summary:

At WeCare, we are seeking a dedicated and compassionate Care Manager to join our team. The Care Manager will act as the primary point of contact for our customers and will be responsible for managing the care teams and care plans. The Care Manager will ensure that all services provided meet the needs and preferences of our customers.

Responsibilities:

  1. Customer Communication, Assessment and Updates:
  • Serve as the primary point of contact for customers, providing regular updates on care plans and services.
  • Conduct initial assessments to identify the needs of senior citizens.
  • Maintain open and effective communication with customers and their families to address any concerns or changes in care needs.
  1. Care Team Management:
  • Manage and coordinate the care teams, including caregivers and healthcare professionals, to ensure the delivery of high-quality care.
  • Schedule and assign caregivers based on customer needs and preferences.
  • Provide guidance and support to the care team, ensuring they have the necessary resources and training to provide excellent care.
  1. Care Plan Management:

Working hours

  • Home care is a 24-hour, year-round service and you’ll usually work between 48 and 54 hours per week. This may include shifts, evening and weekend work, occasional sleep-ins and some on-call duties in the event of an emergency.

Requirements:

– Bachelor’s degree in a related field (e.g., healthcare management, social work). MBA/ MSW in social work is a plus.

– Minimum 2 years of experience in care management or a related role. Freshers are also welcome.

– Strong organizational and communication skills.

Job Summary:

At WeCare, we are currently seeking a compassionate and skilled Care Nurse to join our team. The Care Nurse will be responsible for administering the Care plans to our customers and reporting any changes or updates to the Care Manager. The care plans will include personal care services, health care services, medical care services, and specialized medical care services.

 

Responsibilities:

  1. Administering Care Plans:
  • Implement and follow care plans for each customer, ensuring that all necessary care services are provided.
  • Collaborate with the Care Manager to make any necessary adjustments to the care plans based on the customer’s changing needs.
  1. Personal Care Services:
  • Assist with daily activities such as feeding, bathing, grooming, dressing, undressing, transfer in/out of bed, and bowel/bladder continence.
  1. Health Care Services:
  • Monitor vital signs including blood sugar, blood pressure, temperature, and respiration.
  • Report any significant changes in the customer’s health status to the Care Manager.
  1. Medical Care Services:
  • Provide pre and post-surgery care under the guidance of a nurse.
  • Perform urine catheterization when necessary.
  • Manage wound care under the supervision of a nurse.
  1. Specialized Medical Care Services:
  • Provide specialized care for customers with specific conditions such as cancer, cardiac issues, stroke, Alzheimer’s, and dementia.

The Driver will be responsible for implementing the care plans including transporting our Care teams to and from the clients homes, and transporting our clients to various medical facilities, ensuring their safety and comfort throughout the journey. The Driver will also assist with the coordination of various care services as part of our comprehensive care plans.

 

Responsibilities:

  1. Transportation:
  • Safely and efficiently transport Care teams to and from their homes and client’s homes.
  • Assist clients with entering and exiting the vehicle, ensuring their comfort and safety.
  • Adhere to all traffic laws and regulations to ensure safety of care teams/ clients/ and others.
  • Maintain a clean and well-maintained vehicle at all times.
  1. Care Plan Coordination:
  • Assist in administering the care plans for clients, as directed by the Care Manager.
  • Coordinate with the Care Manager on any changes or updates required in the care plans.
