Job Summary:
At WeCare, we are seeking a dedicated and compassionate Care Manager to join our team. The Care Manager will act as the primary point of contact for our customers and will be responsible for managing the care teams and care plans. Responsibilities:

- Serve as the primary point of contact for customers, providing regular updates on care plans and services.

Working hours

Job Summary:
At WeCare, we are currently seeking a compassionate and skilled Care Nurse to join our team. The Care Nurse will be responsible for administering the Care plans to our customers and reporting any changes or updates to the Care Manager. The care plans will include personal care services, health care services, medical care services, and specialized medical care services.
Responsibilities:
- Administering Care Plans:
- Implement and follow care plans for each customer, ensuring that all necessary care services are provided.
- Collaborate with the Care Manager to make any necessary adjustments to the care plans based on the customer’s changing needs.
- Personal Care Services:
- Assist with daily activities such as feeding, bathing, grooming, dressing, undressing, transfer in/out of bed, and bowel/bladder continence.
- Health Care Services:
- Monitor vital signs including blood sugar, blood pressure, temperature, and respiration.
- Report any significant changes in the customer’s health status to the Care Manager.
- Medical Care Services:
- Provide pre and post-surgery care under the guidance of a nurse.
- Perform urine catheterization when necessary.
- Manage wound care under the supervision of a nurse.
- Specialized Medical Care Services:

The Driver will be responsible for implementing the care plans including transporting our Care teams to and from the clients homes, and transporting our clients to various medical facilities, ensuring their safety and comfort throughout the journey. Responsibilities:
- Transportation:
- Safely and efficiently transport Care teams to and from their homes and client’s homes.
